Although we have extended walk in service capabilities, appointments are required for some of our services. 

Diabetes visits require a referral and appointment.

The Following services require appointments:

1- Urine Drug Screens.

2- Breath Alcohol.

3- Pre employment Physicals (audiometry, spirometry, ECG).

4- Sport Physicals.

The Diabetes Center requires a Referral Form from your primary care physician. Our office can be contacted for our form, or a copy may be printed from this website.
